Gender Affirming- Vaginoplasty

What are the criteria for the procedure?

  • Persistent, well-documented gender dysphoria;

  • Capacity to make a fully informed decision and to consent to treatment;

  • Be the age of majority in a given country;

  • Psychiatric Evaluation

  • Twelve continuous months of hormone therapy as appropriate to the patient’s gender goals (unless the patient has a medical contraindication or is otherwise unable or unwilling to take hormones);

  • Twelve continuous months of living in a gender role that is congruent with their gender identity.

What is the procedure like? 

  • During most vaginoplasties, your surgeon will use a skin graft to create a new vaginal canal (the inside wall of the vagina). To do this, your surgeon will take skin from your scrotum and thin it so it works well as a skin graft. 

  • If there’s not enough skin from your scrotum to make your new vagina, then your surgeon can take extra skin from the sides of your abdomen where there won’t be a very noticeable scar.

  • To make your new vaginal canal, your surgeon will create a space between your rectum and bladder. Once your skin graft is inserted, your surgeon will place gauze or spongy material inside the new vaginal canal for 5 days. The gauze puts pressure on the skin graft so it grows like it should into the surrounding vaginal tissue​

Voice Feminization

Voice feminization/masculinization surgery is a procedure to raise/lower the pitch of your voice or make it sound higher/lower. Surgery changes the length, size or tightness of your vocal cords.

Your vocal cords (also called vocal folds) are bands of soft tissue in your larynx (voice box). They control the pitch of your voice. They vibrate when you take in air. Surgery changes the folds so they vibrate differently and make higher sounds when you speak.

What is the procedure like? 

  • You receive general anesthesia. This is a medication that puts you to sleep so you don’t feel pain during your surgery.

  • Your surgeon may use a laryngoscope to access your voice box. This is a thin, lighted tube that goes into your mouth and reaches back to your larynx. But some procedures require incisions (cuts) on the outside of your throat.

  • Depending on the type of surgery you have, your surgeon does the following:

  • Anterior glottal web formation: Removes several layers of tissue from the vocal folds at the front of your voice box. Then they use stitches to join the vocal folds, shortening the length of your voice box.

  • Cricothyroid approximation (CTA): Stitches together cartilage from the top and bottom of your larynx.

  • Laser reduction glottoplasty (LRG): Destroys the outermost tissues of the vocal cords with light energy from a laser.

Mastectomy

Gender-affirming mastectomy is a type of surgery that removes breast tissue, tightens the skin, and can adjust nipple placement to provide the desired results of a more masculine-looking chest. Mastectomies started as a way for surgeons to remove tumors from the breast tissue.

Gender-affirming mastectomies involve operating on the muscles and tissue of the chest wall and surrounding areas to create a chest that looks more masculine. On the outside of the body, the breasts and the chest wall include the skin, nipples, areolae, and inframammary fold.

Calfplasty

The shape of the calf depends largely on genetic characteristics, but muscle deformity can also occur due to an accident. This problem affects not only the leg but the overall appearance and can even cause a lack of self-confidence and self-esteem.

During calfplasty under anesthesia the surgeon makes the surgical incision in the knee flexion so that the scars will not be noticeable later, then inserts the implant through this 4 cm long incision.

Anatomical implants are usually recommended for men, spindle-shaped ones are generally recommended for women.

Gender Affirming - Phalloplasty

What are the criteria for the procedure?

What is the procedure like? 

Phalloplasty is surgery for masculinizing gender affirmation. Phalloplasty is a multistage process that may include a variety of procedures, including:

  • Creating the penis

  • Lengthening the urethra so you are able to stand to urinate

  • Creating the tip (glans) of the penis

  • Creating the scrotum

  • Removing the vagina, uterus and ovaries

  • Placing erectile and testicular implants

  • Skin grafting from the donor tissue site 

Gender affirmation surgery is customized to each individual.Phalloplasty involves using skin flaps, which are areas of skin moved from one area of the body to another. The skin flap is then reshaped, contoured and reattached to the groin to create the penis.

Thyroplasty

Adam’s apple reduction surgery consists in reducing and contouring the thyroid cartilage to aesthetically look more feminine.

Through thyroplasty or thyroid cartilage shaving (Adam’s apple shaving), we decrease its size. It involves making a minimal incision at the level of a cervical crease to hide the scar and carefully shave the thyroid cartilage.

When it is done alone, it can be done under local anesthesia and light sedation.

Pectoral Implants

Pectoral augmentation — or “pec implants” — adds definition and contour to your chest. It can be used as:

  • A reconstructive surgery after an injury or illness

  • To combat lackluster results in the gym

  • As a female-to-male gender confirmation surgery

Successful pec augmentation uses a silicone gel implant to extend the depth of your muscle so it appears more chiseled and defined.

​

This surgery focuses on your upper chest where your pectoral muscles are, right over your breastbone.

The pectoralis major is a fan-shaped muscle that overlaps the tissue over your heart and part of your rib cage and extends back toward your armpit.
